Nothing impactful comes in siloes.
AmericaServes is a first-of-its kind, market-ready coordinated network of service providers who are self-organizing as a collective body – above and beyond their own unique organizational designs – to ensure unequaled access to the very best and most comprehensive network of services, resources and care designed exclusively for service members, veterans and their families found anywhere in America. Central to the AmericaServes solution is the concept of “Collective Impact”, and the idea that true change comes from community-wide buy-in.
Designed, deployed and funded privately through Syracuse University’s Institute for Veterans and Military Families (IVMF) and its strategic partners, Accenture and Unite US, AmericaServes is transforming the means by which American communities deliver services, resources and care to those who serve their country and those who support them.
Many communities begin at a fragmented state, where providers speak to a difficulty in caring for a client’s full service needs, and those same veterans are likewise unable to receive the right care within a reasonable amount of time. At AmericaServes, providers join a local network and immediately become a part of our coordinated care platform and community of practice. As more and more providers join and become active in the network, the impact to veterans becomes stronger and stronger.
Wherever you are along the path to coordinated care, we can help…
With more than 800 providers in the network and actively using the platform to facilitate referrals, we are coordinating care in 16 communities around the country.
Our three supporting business lines are dedicated to developing community-based practices to best-serve military connected members and their families. AmericaServes provides the insights & analytics, network support, and community workshops (something we call Practice 360) to better readying communities to deliver care.
