As part of our focus to grow and deepen our impact across the country, we are providing a portfolio of materials, guides and training to serve you as Coordination Centers. You are our partners in this quest to serve and to improve lives and outcomes. As such, you serve as the hubs of our networks, and we need you onboard on all fronts – intake, triage, referrals, provider engagement, and community involvement in order to better serve our providers and in doing that our veterans.
In order to ensure network performance, accountability and transparency, the Leadership team, led by the IVMF, has established a series of recommendations and one-pagers to ensure services, resources and care made available through the network remain focused on creating a quality experience for consumers and providers alike. Your input is so important in shaping the Community of Practice that this work represents.
