Best Practice Share – Person-Centered Skills: 4 + 1 Questions

4 + 1 Questions can be used to focus on learning and act on learning. The first 4 questions focus on looking back at what has been tried to solve a problem and the + 1 focuses on looking ahead on what we can do to address the problem. During our recent Community of Practice Mini Conference we used the questions to work with a provider scenario that focused on engagement. The questions can also be helpful to use during focus groups or Local Community of Practice calls when working with several providers in the community to address a particular practice-related issue the network is dealing with. The questions are simple and can open up dialogue around problem solving.
