What is Care Coordination?
Care Coordination is taking community collaboration to the next level. Enabled by Unite US technology and the human capital of a local human service provider, practitioners can refer veterans, servicemembers and families into the network of service providers and know that their needs will be met on the other end. Once a referral is made, the case worker will be able to track the progress and make sure their clients are receiving proper wrap-around services.
How will they know?
Four licensed social workers staff the Coordination Center and it is their job to send referrals to the proper referral. This eliminates the “wrong door” for the customer and the provider on the other end know they are receiving a client who qualifies for their services.
